Services Built Around the Person

We provide 245D services based on assessed needs, person-centered planning, and authorized service agreements. Our team partners with individuals, families, and case managers to deliver safe, consistent, and respectful support.

Basic Support Services

Basic Support Services are designed to promote health, safety, and community participation through supervision, assistance with daily routines, skill support, and caregiver relief—delivered in the home and/or community according to the person’s support plan.

24-Hour Emergency Assistance

On-call support and response coordination for health and safety needs, including planning for emergencies and follow-up communication.

Adult Companion Services

Supervision and support that promotes socialization, safety, and participation in home and community activities consistent with the person’s goals.

Homemaker

Assistance with household tasks that support health and safety such as light cleaning, meal preparation support, and routine household organization (as authorized).

Individual Community Living Supports (ICLS)

Training and support to build skills for more independent living, including routines, problem-solving, communication, and community participation.

Individualized Home Supports (IHS)

One-to-one support in the home or community to help achieve goals related to daily living skills, safety, behavior support strategies, and participation in typical activities.

Night Supervision

Overnight supervision to support safety, routines, and appropriate assistance when a person wakes during the night (awake or asleep staffing as authorized).

Respite Care (In-Home or Out-of-Home)

Short-term care that provides relief to the primary caregiver while ensuring the person receives safe supervision and support according to the support plan.

In-Home Support Services

In-Home Support Services focus on teaching, coaching, and building skills—either directly with the person, or through structured training for caregivers—based on service outcomes and documentation requirements.

Individualized Home Supports with Training

Skills training and structured teaching in the home to support greater independence, safety, and daily routine success, aligned with measurable goals.

Individualized Home Supports with Family Training

Coaching and training for family/caregivers to strengthen consistent support strategies, routines, and skills that help the person meet outcomes safely at home.

Employment Services

Employment Services help individuals explore work interests, prepare for employment, and maintain success in competitive, integrated community jobs. Supports are coordinated with the person’s team and service plan.

Employment Development Services

Support to identify employment goals and secure job opportunities, including job search assistance, application support, and coordination with employers (as appropriate).

Employment Exploration Services

Time-limited activities to explore career interests and build readiness through job-related learning, community-based exploration, and identifying strengths and preferences.

Employment Support Services

Ongoing supports to help a person learn job tasks, maintain employment, and build workplace success using individualized strategies and progress documentation.

Services are provided based on assessed needs, service authorization, and Minnesota DHS 245D requirements. Availability may vary by county and staffing.
